Overview

The coracoclavicular ligament is the main suspensory link between the clavicle and scapula. It has two distinct parts—the conoid and trapezoid ligaments—which work together to stabilize the acromioclavicular (AC) joint.

Location

Both parts run from the coracoid process of the scapula to the underside of the clavicle.

Attachments and Structure

The more medial conoid ligament is cone-shaped. The more lateral trapezoid ligament is flatter and angled. Their different orientations help control motion in several directions.

Function

They resist excessive vertical and horizontal displacement between the scapula and clavicle, helping the arm and shoulder blade remain suspended from the collarbone.

Common Injuries

These ligaments may be sprained or torn during an AC joint separation, usually after a fall directly onto the outer shoulder. The degree of displacement depends on which supporting ligaments are damaged.

Clinical Relevance

Examination and X-rays help grade an AC injury. A visible bump does not always predict pain or function, and many lower-grade separations recover without surgery.
