Overview
A shoulder that feels tight, stuck, or won't move through its usual range is a common and often frustrating symptom. It can show up gradually over weeks or months, sometimes without any clear injury at all. Stiffness is different from weakness — with stiffness, the joint itself resists moving, almost like it's being held back, rather than the muscle simply lacking power.
What This Symptom Feels Like
- Difficulty reaching behind your back, like doing up a bra or tucking in a shirt
- Trouble reaching overhead to grab something from a high shelf
- A "stuck" or "tight" feeling rather than sharp pain, especially at the end of the movement
- Reduced motion that's noticeable in more than one direction (not just one specific reach)
- Stiffness that's often worse first thing in the morning and eases slightly with movement
- A gradual, creeping loss of motion over weeks to months, rather than a sudden change
In plain terms: your shoulder socket joint (the glenohumeral joint, where your upper arm bone meets your shoulder blade) is surrounded by a flexible capsule and ligaments that normally let it move more freely than almost any other joint in your body. When that capsule tightens, thickens, or the surrounding tissues become irritated, the joint's normal glide is restricted — so instead of moving smoothly, it feels tight or stuck.
Common Causes
- Adhesive capsulitis (frozen shoulder) — the joint capsule thickens and tightens, causing progressive, often significant stiffness
- Shoulder osteoarthritis — wear-related changes in the joint surface can limit motion, especially in older adults
- Rotator cuff tear — pain from a tear can lead to guarding and gradual stiffness over time
- Shoulder impingement syndrome — ongoing pain with movement can cause people to avoid certain ranges, which itself leads to stiffness
- Acromioclavicular (AC) joint injury — can restrict motion, particularly reaching across the body
- Subacromial bursitis — inflammation can make certain ranges feel tight and guarded
Stiffness often develops as a secondary effect: pain from almost any shoulder condition can make you protect the joint by not moving it fully, and after a while, the tissues themselves adapt to that reduced range and become genuinely tighter.

- Is the stiffness limited to one direction (like reaching behind your back), or does it affect motion in several directions?
- Did the stiffness come on gradually over weeks or months, or was there a specific injury?
- Is the stiffness worse in the morning and does it loosen up somewhat during the day?
- Is there significant pain along with the stiffness, or is it mostly a "stuck" feeling?
- Has the stiffness been getting steadily worse, staying the same, or slowly improving?
- Do you have any other joint conditions (like diabetes or thyroid issues), which are linked to a higher chance of frozen shoulder?
- Can someone else move your arm for you further than you can move it yourself?
When It's Usually Minor
Mild, temporary stiffness — especially first thing in the morning that loosens up with gentle movement — is often just a sign of a shoulder that needs to warm up or one that's been guarding against recent irritation. If the stiffness is mild, not worsening, and doesn't stop you from most daily tasks, it often responds well to gentle, consistent movement.
When To Seek Medical Care
- Stiffness that is progressively worsening over several weeks, especially with significant pain
- Near-total loss of motion in multiple directions
- Stiffness following a fall, dislocation, or other trauma
- Stiffness combined with numbness, tingling, or noticeable weakness
- Signs of infection — redness, warmth, fever, or a hot, swollen joint
- Stiffness that is significantly affecting your ability to work, sleep, or manage daily tasks
Self-Care Options
- Keep the shoulder gently moving rather than avoiding it completely — motion is usually better than complete rest for stiffness
- Try pendulum exercises as a low-effort way to encourage movement without forcing it
- Apply heat before movement to help tissues loosen, and ice afterward if there's any irritation
- Be consistent rather than aggressive — small, frequent range-of-motion sessions tend to work better than occasional intense stretching
- Pay attention to posture during long periods of sitting, since a rounded, forward posture can contribute to tightness
- Give it time — stiffness related to frozen shoulder in particular can take months to improve, and progress is often gradual
